The Case Management Program provides a comprehensive care program that addresses the often complex needs of individuals and families living with HIV and AIDS.
All individuals living with HIV disease are eligible for case management services and medical care at AIDS Community Services regardless of ones ability to pay. No one is ever charged for case management services. If you find that you or a loved one is in need of these services, enrolling in services at AIDS Community Services is easy and completely confidential. Just call or come into any one of our offices (for locations and contact information, click here) and our staff will take care of you on the spot.
The initial intake and screening will include a brief interview and intake process to collect pertinent information. A case manager will then conduct an assessment which includes identifying the client’s strengths, resources, problems and needs along with a review of their clinical care, substance use, TB history, etc. to determine their utilization and adequacy of medical care. They will then work with the client to identify any existing barriers to care and existing gaps, evaluate service needs and problems, and ask the client about their support system (ie: family friends that may be able to assist and support the client).
The case manager will update the above information with the client support system every 180 days. The case managers role is to work closely with the client and their family to assist them in obtaining any and all assistance they may need in their quest to stay healthy.
With the case manager, the client will identify their individual short- term and long-term goals. They will also identify services, specific interventions, and activities necessary to achieve these goals. Provide comprehensive, ongoing assistance to individuals with HIV/AIDS, as well as those considered at-risk for infection.
Coordinates medical treatment, housing, entitlements, mental health, chemical dependency, and other services in order to meet client needs.
Makes appropriate referrals to outside agencies for services as needed by client.
Provide services to families, partners, children, and others in addition to the client in order to reach all of those who have been affected.
Advocate on behalf of clients with providers to insure proper treatment and care.
Has offices in downtown Buffalo, Jamestown and Niagara County serving Erie, Niagara, Genesee, Wyoming, Chautauqua, Allegany, Orleans and Cattaraugus Counties.
Offers most services in English and Spanish.
